Information we collect

• Information you submit on a form — such as your name, email address, phone number, a requested appointment time, and anything you choose to write in a message.
• Whether you have chosen to receive appointment text messages, and, if so, the exact consent language shown to you and the time you consented.
• Standard technical information your browser sends automatically, such as device and browser type and general usage data, used to keep the site working and secure.
• Which pages of this site you visit, measured with Google Analytics — see “Website analytics” below for exactly what we send and what we withhold.

Website analytics

We use Google Analytics to understand which pages of this public website people read, so we can tell whether the information here is useful and easy to find. Google sets cookies on your browser to do this and processes the data on our behalf as a service provider.

We deliberately limited what Google receives. We send the page address, the page title, and the page you arrived from. We strip anything after the address itself — the parts of a link that can carry a name, an email address, or an appointment detail are never sent to Google. We send no information you type into any form on this site: not your name, your email address, your phone number, your requested time, or anything you write in a message. We have turned off Google Signals, advertising features, and ad personalization, so this data does not follow you around the internet or build an advertising profile.

You can block this entirely. Most browsers let you refuse cookies, and Google publishes a browser add-on that opts you out of Google Analytics on every site. Refusing analytics does not affect your care, your ability to request an appointment, or anything else on this site.

Analytics runs on these public pages only. It plays no part in your medical record, and nothing a patient shares in the course of care is sent to Google — see “Health information” below.

How we use your information

• To send you appointment-related communications by email or phone and, only if you opt in, by text message (see our Text Messaging Terms).
• To operate, secure, and improve this website, and to meet legal obligations.

Text messaging

Appointment text messages are optional and require your affirmative opt-in. You can stop at any time by replying STOP, and get help by replying HELP. The terms of the program are described in our Text Messaging Terms.

We do not share, sell, or provide your mobile phone number or your text-messaging consent data to third parties or affiliates for marketing or promotional purposes. We do not sell your personal information. Mobile opt-in information is used only to operate the appointment text-message program.

How we share information

We do not sell your personal information. We share it only in limited ways: with service providers who help us operate the website and our scheduling, messaging, and website analytics (for example, our website host, our text-message provider, and Google Analytics), under agreements that limit their use of it to providing those services; and when required by law or to protect safety. Any use of your information to deliver appointment text messages is limited to that purpose, and nothing you type into a form on this site is sent to our analytics provider.

Health information

Protected health information created or received in the course of your care is governed by federal and state health-privacy law — including HIPAA and, where applicable, additional confidentiality protections for certain records — and by the practice’s Notice of Privacy Practices, which is provided to patients and available on request from the office. This website Privacy Policy governs the website; it does not modify your rights or our obligations under the Notice of Privacy Practices. Information you submit through this website to request or obtain care may itself be protected health information; where it is, we handle it consistent with HIPAA and the practice’s Notice of Privacy Practices. Please do not include sensitive medical details in a website form or a text message.

Data security & retention

We use reasonable safeguards to protect the information you submit and retain it only as long as needed for the purposes above or as required by law. No method of transmission or storage is completely secure.

Children’s privacy

This website is intended for adults and is not directed to children under 13, and we do not knowingly collect their information through it.
